Beta-Blockers and Coronary Flow Reserve
نویسنده
چکیده
The knowledge of the impact of beta-blockers on Coronary Flow Reserve (CFR) is based on experimental and clinical studies which used invasive methods (mainly Doppler Flow Wire, DFW) and non-invasive tools including scintigraphic (mainly Positron Emission Tomography, PET), magnetic resonance and Doppler ultrasound imaging. Beta-blockers have a large therapeutic indication in the treatment of coronary artery disease, due to their anti-ischemic and anti-arrhythmic effect. The anti-ischemic effect is based on the oxygen sparing mechanism with a reduction in rate-pressure product. Changes in coronary hemodynamics associated with the administration of beta-blockers have been extensively studied. These drugs may affect CFR by modifying either resting or maximal coronary blood flow (CBF) or even both. When assessing the impact of beta-blockers on CFR it is important to distinguish the effects exerted by first-and second generation beta-blockers from those due the mechanisms of third generation beta-blockers, which are provided of vasodilating action.

Different effects of carvedilol, metoprolol, and propranolol on left ventricular remodeling after coronary stenosis or after permanent coronary occlusion in rats.
BACKGROUND Although carvedilol attenuates left ventricular (LV) remodeling in coronary occlusion-reperfusion, it is not known whether it attenuates ischemic LV remodeling because of coronary stenosis (CS) or permanent coronary occlusion (CO). METHODS AND RESULTS We administered a vehicle, carvedilol, propranolol (2, 10, and 30 mg/kg per day, each), metoprolol (6, 30, and 90 mg/kg per day), or...
متن کاملAbsolute flow or myocardial flow reserve for the detection of significant coronary artery disease?
OBJECTIVES We compared the accuracy of quantified myocardial flow reserve and absolute stress myocardial blood flow (MBF) alone in the detection of coronary artery disease (CAD). BACKGROUND Myocardial flow reserve, i.e. ratio of stress and rest flow, has been commonly used to detect CAD with many imaging modalities. However, it is not known whether absolute stress flow alone is sufficient for...
متن کاملRecurrent angina after coronary angioplasty: mechanisms, diagnostic and therapeutic options.
Recurrent angina in patients who underwent percutaneous coronary intervention is defined as recurrence of chest pain or chest discomfort. Careful assessment is recommended to differentiate between non-cardiac and cardiac causes. In the case of the latter, recurrent angina occurrence can be related to structural ('stretch pain', in-stent restenosis, in-stent thrombosis, incomplete revascularizat...
متن کاملCoronary physiology revisited : practical insights from the cardiac catheterization laboratory.
Various coronary physiological measurements can be made in the cardiac catheterization laboratory using sensor-tipped guidewires; they include the measurement of poststenotic absolute coronary flow reserve, the relative coronary flow reserve, and the pressure-derived fractional flow reserve of the myocardium. Ambiguity regarding abnormal microcirculation has been reduced or eliminated with meas...
متن کاملThe Vulnerable Plaque: the Real Villain in Acute Coronary Syndromes
The term "vulnerable plaque" refers to a vascular lesion that is prone to rupture and may result in life-threatening events which include myocardial infarction. It consists of thin-cap fibroatheroma and a large lipid core which is highly thrombogenic. Acute coronary syndromes often result from rupture of vulnerable plaques which frequently are only moderately stenosed and not visible by convent...
